A Message from Campolindo High School Parents Club & Campo Sports Boosters:
Get your 2016-2017 Athletic Clearance for Fall, Winter and Spring Sports at Campolindo High School
Sports Physicals will be offered on 2 dates (physicals will take place in the multi-use room):
- Thursday, July 21st from 6:30-8:30 PM
- Tuesday, August 16th from 6:30-8:30 PM (Cougar Day)
Any student participating in Campolindo Athletics during the 2016-2017 school year may obtain their sports physical and athletic clearance at Campolindo High School…and support Campo Athletics at the same time!
Several area doctors, nurses and health professionals have volunteered their time to examine Campo students and provide athletic clearances on 2 dates this summer; 7/21 and 8/16.
Registration is simple and the cost is just $40.
1) Fill out an Athletic Clearance form available on the AUHSD website at http://www.acalanes.k12.ca.us/athleticregistration. Bring form with payment to the physical.
2) Payment Options:
For Thursday, July 21: Bring a $40 check made payable to Campolindo Sports Boosters along with your district form.
For Tuesday, August 16: Pay via the webstore during “Online Cougar Day” registration. (Bring your receipt for proof of payment and your district form.)
This is a major fundraising activity for Campo Sports Boosters; all proceeds go towards the funding needs of Campo’s sports programs.
REMINDER: Athletic registration and the district form MUST be obtained via the AUHSD website. You can access the site and obtain the form during “Online Cougar Day” registration and follow the prompts to complete the process OR visit the district website at http://www.acalanes.k12.ca.us/athleticregistration. Please complete the form and bring it to your sports physical.
