Get your 2012-2013 Athletic Clearance for Fall, Winter and Spring Sports at Campolindo High School and Support Campo Sports Boosters

Sports Physicals Available

 Monday July 16, (5:30 – 7:30 pm)

Or Wednesday August 15th (6:00-8:00 pm) 

If your student is participating in Campolindo Athletics during the 2012-2013 school year and needs an athletic clearance; as an alternative to visiting your family physician your student may obtain their physical and athletic clearance at Campolindo High School on either Monday July 16 and Wednesday August and support Campo Athletics at the same time! Several area doctors, nurses and medical field professionals will be volunteering their time (again this year) to examine Campo students and provide athletic clearances. This is a major fundraiser for Campo Sports Boosters and all proceeds go towards covering the funding needs of Campo’s sports programs. See details below for the two available times. 

Monday July 16, 5:30-7:30, Multi-purpose room

• Bring a $35 check made out to Campo Sports Boosters
• Go to the Acalanes School District site. Click here to go to the AUHSD site. Complete the Athletic Registration and Form Process and bring the completed Atheletic Clearance form with you on July 16 for the doctor to sign
• Complete Page 1 of the Preparticipation Physical Evaluation form (click here for form)

Cougar Day, Wednesday, August 15th, 6:00 - 8:00pm, Multi-purpose room

For football players, this option is only available for Freshman due to timing of the JV and Varsity practice

• Visit the Webstore on the Campolindo High School web site and pay $35 during registration week. Print out a receipt and bring with you to Cougar Day. OR bring a $35 check made out to Campo Sports Booster with you on August 15.
• Go to the Acalanes School District site. Click here to go to the AUHSD site. Complete the Athletic Registration and Form Process and bring the completed Atheletic Clearance form with you on July 16 for the doctor to sign
